The packet includes:
- New Patient Information Form
- Medical Records Release Form (for retrieval of your past medical records)
- PHQ-9 and GAD-7 Screening Form
- Allergy Questionnaire and Order Form
Before Your Appointment
Please bring the following with you to your visit:
- A valid photo ID
- Your insurance card (if applicable)
- A printed or completed copy of the registration packet
